Children and Young People’s Mental Health Services (8-25 Years Old)

Working with a local mental health provider we are able to support children and young people as they find their way through the challenges life throws at us all.

Our young people are offered appointments outside of school hours.  The appointments can be face to face, on the telephone or via a video call, whichever method suits their communication style best.

Each young person that we work with is asked to provide a statement of where they are before treatment and then again as their treatment draws to a close.  By undertaking this work we can see that significant improvements have been made to many of our of children and young people.

If you, or someone you know would like support please contact your registered practice and ask about a referral to the PCN’s Children and Young People’s Mental Health Services.

Adult Mental Health Services (Over 18’s)

Our PCN Mental Health Practitioner is available to offer support to our adult population through appointments delivered within our member practices. 

Our Mental Health Practitioner is supported by the local NHS mental health provider giving her access to highly skilled therapists should a patient’s needs exceed her experience level.

Our Mental Health Practitioner has the ability to onwards refer adult patients for Talking Therapies via IAPT or to the Clinical Psychologist affiliated with our PCN.

Podiatry

Janine, our Watling Street Primary Care Network Podiatrist involves diagnosing, treating, and managing complex foot problems that arise as a result of musculoskeletal, inflammatory, diabetic or vascular disease.
 
Janine can also offer minor surgery for ingrowing toenails and can also perform acupuncture for many foot conditions.
 
Podiatrists are placed to use their expertise in primary care settings by developing and embedding services that extend the ability of GPs and primary care teams to provide a focus on prevention and early intervention.
 
The benefit of having a Podiatrist in our PCN include:
- Reduced waiting times
- Free up GP appointments
- Better communication within primary care; assessments can be directly documented within patients notes, rather than waiting potentially weeks for a clinic letter to be sent for further management
- Improved patient satisfaction related to shorter waiting times and reduced likelihood of hospital visits

Physiotherapy

Primary Care Cancer Personalised Care Nurse Facilitator

Cancer Care Nurse Anne is working in conjunction with Macmillan support.
 
Anne will review and support patients with a cancer diagnosis throughout their first year and beyond, helping people live a fuller life with their condition. Anne can be a link between you and your GP giving you easier access to support in the community.
 
Anne can give holistic support including:
- Financial
- Emotional
- Physical
- Social
 
Quick access can be gained to other health professionals including:
- Social Worker
- Occupational Therapist
- Social Prescriber
 
You will receive a letter inviting you for a cancer care review and guidance on how to book your appointment, if it is an inappropriate time during your treatment please let us know that and we can call you at a later date.
