Want to make a complaint?
We make every effort to give the best service possible to everyone who attends our PCN.
However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.
To pursue a complaint please complete this form and the complaints manager will deal with your concerns appropriately.

Once you submit your complaint, please accept this letter as acknowledgement, we will investigate the points you have raised and get back to you with an explanation.
Whilst there are no timescales for providing a response, we will aim to get back to you within 30 working days, if this is not feasible you will be updated, and informed of the reasons for the time taken, in writing if there is no response provided within six months.
